If you click on your audiogram link and mouse over the diagram outside of the hearing loss lines, it shows you what the different ranges are. Based on this, I think yours would be classified as moderate to severe from 750 hz on up.
Side note: Not an audiologist here but I have worn a hearing aid for 42 of my 45 years.

Definitely moderate to severe. You need Rx aids. OTC ones won’t be strong enough. My loss is similar to yours.
My loss is moderate to severe and I hear as well with my Sony OTC aids as I do with my well tuned Phonak Lumity. I’m wearing them now. YMMV
My audiogram (see in bio) is what is called mild to moderate hearing loss, I believe. I don’t wear hearing aids at the moment.
