What audiogram is best served by power domes?

I am a DIYer with Oticon Intent 1’s. I have both Power Domes and Double Vent Bass domes. My difficult situations are like everybody else’s - groups. I’m in groups three to four times a week and in comfortable situations the rest of the time. I suspect I will gain understanding in groups with Power Domes, but am worried I will lose something at home.

Yeah so you could simply set up 2 different fittings for each
(Probably not much difference between those 2 anyway) or just change the domes and do some adjustments through the App, sounds like some custom molds would probably be more of a long term solution.

Hi John, I am also a DIYer and my audiogram is similar to your. I am also using Intent 1 aids with 85dB receivers and power dome with single vent. I am also reluctant to using double bass due to occlusion effect and having moist ears.

I have found the Intents to have had greatest incremental improvement for speech clarity compared to any of the Oticon’s previous models upgrades.

I also use the standard Oticon protocol with speech rescue enabled. Let me know if you have any other insights and adjustments you might have found that helped you and I will do the same.

Hello John,

With your level of hearing, Oticon’s excellent feedback management technology should allow you to use the OpenBass dome as well. Judging by the pattern of your hearing, I believe your speech discrimination should be good. However, if you receive proper real ear measurement fitting from a skilled audiologist, you can expect even better results.

I have been happy with the double vent bass domes. The power domes give me too much occlusion. I have left speech rescue turned off because it seems to distort music for me, but after reading your comment, I may try turning it back on and seeing if I “get used to it.” I am looking for that trade space between being better able to hear conversations and not having distorted sounding audio on the TV or music on the speaker.
In general I would say the Intents are the best aids I have used.

Similar/different hearing loss here.

I’ve been using Closed domes with two tiny vents for quite some time. I’ve had issues with my Phonak Paradise P90R’s since I got them. Setup issues–they should be better than they are.

People here have been most helpful, but I’ve been stubborn. I’ve finally ordered c-shells and am waiting for them.
I have Power Receivers. I’ve had Power Domes a number of times. I really dislike them.

My dispensing audi tried his best, then fired me.

At first glance, your audiogram does not shout “power dome”, and your following comment about occlusion with power domes is consistent with the first pitfall I would suspect when considering them for a patient with a hearing loss configuration similar to yours. Oticon’s bass domes would be an appropriate first choice, but a hollow sleeve mold (LiteTip for Oticon) or a deep fitting embedded receiver mold (MicroShell for Oticon) with a vent would be a second option for me. However, without looking in your ears, I think bass domes should be sufficient.

I don’t love frequency transposition (speech rescue) for the same reasons you have shared regarding sound quality, but with parameters backed off it may be beneficial in a secondary program.

I meant to say I use the bass domes with single vent and I am reluctant to us the power domes to avoid occlusion.

I have found the speech rescue to really help with discrimination of speech for the F, K, S, Sh, Th region. Initially everybody seems to talk with a lisp but after a while with adaption of brain hearing it eventually all seems to sound quite normal.
