How to translate Costco Hearing Test into HearingTracker format

I want to update my Audiogram Chart on HearingTracker but I have forgotten how to translate Costco chart format into HT’s format since the last time I did it… Any (simple) tips would be appreciated.

If you click on the word “FORUM” at the top of this screen :
then click on “My Hearing Tests” … then click on “Edit” … then enter the new values for your left and right ear for each frequency . … easy peasy .

Sorry I did not make myself more clear. My problem is not how to enter the values onto the Hearing Tracker format.
My problem is how to find those values on the Costco chart:

The bottom row (x-axis) is the frequency in khz. The vertical column (y-axis) is the level.
Follow the blue x’s for the left. The red o’s for the right. The other symbols are kind of optional. The mid-points are simply like 65 etc for that first blue mid-point.

.25 is 250HZ .5 is 500 1 is 1000hz and so on .

so left ear 250 is 60 db … 500 is 65 db.
