Domes/tips don't maintain position in ear canal - solutions?

YES. I also envy them. I don’t know exactly HOW they achieve this miracle, but for me, a big improvement was when I swapped (L) ear wires (attaching the speaker to the BTE unit) with (M) ones. They are just that tad shorter so I no longer have this goofy loop poking out of my ears.

Even so. I’m convinced that dry or oily, the ear canal is going to extrude ANY mold of ANY shape, style or material. I’ve had a “leash” attached to my speaker. Doesn’t matter, the dome works its way out of my ear canal and while the leash is still in place, dang dome is at the doorway causing poor sound quality.

I’ve had custom molds on ITE models, custom acrylic and firm silicone molds on BTE models, and no matter what I’ve tried, whatever goes in my ear canal works its way OUT allllllllll day long. So I just push the speakers back in allllllllll day long, like pushing glasses up your nose bridge.
