Adjust hearing aid to audiogram

Had real ear originally.
Distortion is damaged nerve in left.
Distortion there aids or no aids.

Achieving 100% target gain (e.g., in NAL-NL2) immediately for HA beginners may give that unwanted effect.

However, I think fitting software has a feature in which an HCP may set, e.g., 70% of the target gain. Secondly, proprietary prescriptions such as those in Phonak Target place more emphasis on comfort than audibility.

Yes. I had evaluation done by ENT. Speech recognition in right ear is 80% at 75dB and left ear is 40% at 85dB

They were only tested individually.

Gordon, did your Real Ear Measurements transfer to the software as there is a separate section that shows that. The Real Ear Measurement should be important in setting up your hearing aids.

I really did not do anything to transfer any settings. I linked the hearing aids to the software and all of the original settings from the hearing aids, including audiograms and additional programs like restaurant transferred to the program. I then modified those settings to get better results. I could do some adjustments on my iPhone but I could not make them part of the default programs. I knew that increasing the midrange gain helped so that was the first thing I tried and I have been adjusting that in small increments. I also increased the volume for soft speech in the environmental settings. This has probably made a difference when trying to hear my wife who insists that I should be able to hear her when she talks softly with her head down.

There is a lot that I do not know or understand about the program. I am a machine designer/builder, not an audio engineer. Most of the terms are not familiar to me and I am not even sure of what I should be looking at.

Way back I heard music better when I increased the volume. Car radios too.
20 years of hearing aid use Iā€™m still concerned that there are fatal flaws in this hearing aid business.
When the supplier finishes we all have the same issues. Understanding speech in noise. And what about enjoying music?.
So I watched countless youtube videos. I read books. Some were recommended here.
I have 3 year old Smart hearing aids. So Iā€™m told. Directional microphones. Saved programs. An App. After warranty replacement they work better than they ever have. Yet we are dependent upon someone setting them right. And no two peopleā€™s solutions are the same.

Positive note? Iā€™m really happy that the manufacturer supports my hearing aids. There have been two new models released since I got mine.

Gordon, it looks like you have a good start on tweaking the hearing aids. The Real Ear Measurement is when the testing person puts a device in your ear along with the hearing aids that measures what frequencies and loss your ear is really hearing compared to the audiogram you took first. Because your physical ear picks up sounds in addition to the hearing aids, the sounds you really hear and frequencies you hear are a bit different than the audiogram that measured only sounds that went deep into your ear during the audio gram. On the software program there should be a tab at the top that says "Real Ear Measurement "or ā€œREM.ā€ I am guessing that it may not have any data in it because the test may have to be run separately by the audiologist or specialist with their machine. The REM is an important measurement and crucial to getting the best accurate results with your hearing aids By the way, you can request a printed copy for both the Audiogram and the Real Ear Measurement Test.

Well in all of the testing that I have done with both the ā€œretail dealerā€ and the ENT the only test that was done was to put me in the soundproof room with headphones and have me push the buttons when I heard a tone. Then they spoke a bunch of simple words for me to repeat.

I do not see any tab on the top of the software program showing REM. I am using the Resound Smart Fit program linked to my Resound Omni 7 hearing aids. At no time did I initiate any program to set my hearing aids to the Audiogram. When I first used the software it read the setting that were already in the hearing aids including the Audiogram. From there I have tried incremental changes to the gain adjustments. I have made these changes by experimenting with my iPhone settings to determine add or subtract from Bass/midrange/treble and then tweaking the program and trying that for a day or so.

I had a case of sudden ā€œidiopathic hearing lossā€ where I lost all hearing in my left ear which was not treated for about a week at which time I was given steroids and the result was about a 40dB permanent loss in that ear. About two years later the same thing happened in my right ear and I was give the steroid after about two days so the loss was much less severe.

These are the frequencies the app uses:

Low - <290 Hz
Mid - 500-2.5 KHz
High - >1.5 KHz

My best results have been small increases in G50 from 1.5 KHz upwards. The app indicated 1 increase in Mids, and 2 increases in High.
