I wonder if the difficulty of programming for speech in noise has to do with this. In the sense that, the issues are more middle ear related.
Heck, for that matter, my ears have been massively reconstructed in order to deal with cholesteotoma issues in my pre teen years. A GP has no idea what he or she is seeing when peering into my ears. all that reconstruction HAS to effect the acoustics happening in my ears.
